All women of child bearing age who have breast reduction or reduction mammoplasty surgery should be warned that breast feeding may not be possible after surgery.
The technique used may have some influence. For example when the reduction is performed by amputation of the nipple and free nipple grafting there is almost no possibility of breast feeding, because all the ducts to the nipple have been destroyed.
When the nipple is preserved on a dermal pedicle, as for example in the McKissock technique, many of the ducts are destroyed but lactation is still possible although unlikely.
Some surgeons feel that the inferior pedicle technique may give the greatest chance of breast feeding because it may preserve the connection between the nipple and largest amount of breast tissue.

- Although it is too early to tell with certainty, breast reduction using liposuction seems more likely to preserve the ability to breast feed and also leaves fewer scars. For younger patients with moderate degrees of enlargement this may be an alternative.
