- Brow lift by excision of the skin above the brow is no longer as popular as it used to be. This is because it leaves a scar which may be visible for a long time and the hairline of the brow may have an unnatural look.
As a general principle the closer the incision is to the structure to be raised the greater the elevation. For this reason care must be taken when performing this operation in men because it can create an unnatural surprised look.
Nevertheless in selected patients direct brow lifting can produce a good result. It is most useful for patients who cannot have a standard forehead lift or in whom an endoscopic approach is not possible. Patients with thin skin or hair, and those with a high hairline are sometimes candidates. Patients who use thick brow pencil to create the brow are also candidates because they can conceal the scar with makeup.
It is important that the elevation on each side is symmetrical. Some surgeons make a pattern using paper or x-ray film so that the same amount is removed on each side.
