BEFORE THIGH LIPOSUCTION Indications: liposuction is done to remove localized areas of fat from many areas of the body. It is often hard for the patient to decide between an abdominoplasty and liposuction.

Liposuction does not tighten skin although sometimes the underlying tissue seems pulled tighter by a thin layer of scar tissue (adhesions).

Liposuction is used to sculpt areas and is not designed to remove large amounts of fat or to treat obesity. A newer technique called ultrasonic liposuction may be able to remove larger amounts of fat.

Cellulite is not usually improved by traditional liposuction and in some cases may be made worse.

Method: The method of surgery is to make small incisions through which the fat is removed in individual tubes.

Different sized cannulae, or suction tubes, are used to remove the fat and the pattern crisscrosses to reduce the risks of irregularity.

Liposuction is done under either local anesthetic (you are awake) or general anesthetic (you are asleep). You can often choose but most patients are are more comfortable asleep. Almost always the area is distended with saline and this is called tumescence or superwet technique.

Usually liposuction is an outpatient operation and a stay in the hospital is not needed.

Problems: All operations have some risk.

The risks of liposuction are divided into two groups. First those that are seen in all operations and second those that are unique or special for liposuction.

In the first group, the main risks are swelling, bruising, bleeding, infection, a scar and numbness or change in feeling.

The main problems which are special or unique for liposuction are surface irregularity and the need for secondary surgery.

Ultrasonic liposuction also has complications such as nerve damage but probably causes less bleeding and irregularity.

Postoperative care: The recovery takes one to two weeks. Most people are back to work within a week.

There is some evidence that large volume liposuction may have beneficial effects in people with metabolic problems, and may help diabetes, obesity and hypertension. Other studies have not confirmed this.

LIPOSUCTION

As liposuction and ultrasonic liposuction become more widely used it is not surprising that larger amounts of fat are removed. Although it is not a treatment for obesity newer techniques have opened the possibility of large volume extraction. When performed more than once it is possible to remove significant amounts of fat. It is not a substitute for diet and exercise and the fat deep in the body will not be affected, only that between the skin and the muscle in the area of surgery.

In general liposuction is very safe. Complications of surgery can occur no matter what the operation and include bruising, swelling, bleeding, infection numbness and the scar. Those specific for ultrasonic liposuction include surface irregularity, the need for secondary suctioning and damage or burns to the skin. However with the increasing use of a large volume extraction has come the recognition of special complications and problems. Although very rare some of these are potentially serious. For this reason large volume extraction should be considered a major surgical operation.

Some potential risks include:

BOWEL PERFORATION:the tip of the suction cannula can penetrate into the bowel or other organs and lead to serious problems.

LOCAL ANESTHETIC TOXICITY: tumescent fluid is essential in large volume extraction and this can lead to excessive doses of local anesthetic (lidocaine).

PULMONARY EDEMA: in large volume extraction large volumes are fluid are administered and this can lead to heart failure and fluid in the lung.

SHOCK: excessive bleeding can lead to serious blood loss.

NECROTIZING FASCIITIS: a serious infection of the tissue underlying the skin can lead to major problems. For this reason antibiotics are usually used.

BLOOD CLOTS: clots can form in the leg and if these break free can cause serious medical problems This is more likely in patients on birth control pills or with a history of previous clotting problems.

FAT EMBOLI: small pieces of fat can break off and circulate around the body causing strokes or lung problems.

Despite these risks there is some evidence that in some patients liposuction may have beneficial effects on metabolism and especially diabetes, fats and high blood pressure.
